We introduce a patient with symptoms of an olfactory reference syndrome (ors), characterized by a strong belief to give out a ghastly smell. This somatic subtype of delusional disorder should be distinguished from other psychiatric and organic disorders. It deserves our medical and therapeutic attention because, without treatment, a serious secondary depression may develop.
